A warranty claim form has one job: give your team enough to make a decision on the first pass. Most claim back-and-forth comes from a missing serial number, an unreadable receipt or a photo of the wrong part. This template fixes that with the right fields, in the right order, with requirements that depend on the claim reason.
Copy the fields into any form builder, adjust the warranty terms to yours, and you have a working claim form today.

What to require per claim reason
Asking for every document on every claim slows down the honest majority. Tie requirements to the reason instead:
| Claim reason | Photos | Video | Proof of purchase |
|---|---|---|---|
| Arrived damaged | Required | No | Required |
| Doesn't turn on | No | Optional | Required |
| Stopped working | Optional | Optional | Required |
| Noise or intermittent fault | No | Required | Required |
| Cosmetic defect | Required | No | Required |
| Missing parts | Required (box contents) | No | Required |
If the product was registered, the registration already holds the purchase date and serial number. Make the receipt optional for registered products and ask for it only when something doesn't match.

Tips that cut claim handling time
- Offer troubleshooting before the form. Many "broken" products are setup questions. A short troubleshooting step or help article before the form keeps those out of your claims queue.
- Show where the serial number is. A photo of the label next to the field removes most wrong or missing serial numbers.
- Use dropdowns, not free text, for models and retailers. It keeps reporting clean, so you can see which products and channels produce claims.
- Send a confirmation with a claim number straight away. Customers who know their claim arrived don't email or call to check.
- Check claims against your records, not just the form. Repeat claims on the same serial number and claims outside the warranty period are the most common problems. Our warranty fraud guide covers the rest.
Skip the form: claims that fill themselves in
A form asks customers for things you could already know. With Veribl warranty claims, customers scan the QR code on the quick-start card or in the manual, and the claim opens with the product, serial number, purchase date and warranty status already filled in from their product registration. They describe the problem, add photos when the claim reason needs them, and your rules route the claim. The AI assistant on the same page answers setup questions first, so fewer claims are filed for problems that can be fixed at home.
